Medics

Do they provide their bonus if kept at Army HQs or do they have to be in the same region attached to a corps?

The manual and the tooltip say "of the stack they are in".
So they have to be, as you said, attached which I take to mean part of the same tab.
A medic in the same stack as an Army HQ would only affect units in that HQ stack, not the entire Army.
You would have to put a medic in each Corps. I did and it was very effective, some of my divisions were getting cohesion recover rates of 1.40 per day.

And...

don't forget you can also put divisions under the control of the Army commander, so the medic will apply to them.

Indeed. Having a medic attached to each of my corps is a "no-brainer" in my strategy book; starting with the main combat corps, and the spreading out to the less significant corps and forces if/when I get the number of medic units up.

Only one bonus per stack. 1 medical unit is just as good as 25.

There is no such thing as "too many" medics... :)
