I tried to build a 'permanent fortification' (4 arty, 2 supply) in a region with a harbor (military control, 100% loyalty) but it wouldn't build. I then tried to build it in a region with rail line (military control, 100% loyalty) but it wouldn't build. I tried the same unit in a city and the fort can build.
Can forts only be built in cities?
BTW, using latest patch...

Yes, cities only I think, but a level 8 entrenchment is a fairly good defensive structure if you can't build a fort.

As the Union I don't know how usefull a fort would even be as you are mostly on the offensive ... I would say that about the only place I would like to build one is in Tucson, because the CSA is very determined to take this and the Union has a very hard time getting anything there to assist the garrison ... I just don't see that I could get so many supply and artillery units there w/out them being intercepted or simply destroyed by starvation as usually happens to me in the Kansas and Indian Territory region.
Why can I not get to Arizona from California ... maybe it should be the only way to get to Arizona so that the CSA AI does not waste so many of its valuable western resources on this outpost.

This is on purpose yes. The forts you can build are extensive field fortifications, like around Washington or to a less extent Visckburg. They are truely exceptionnal. For the rest, the high level of entrenchment are very good and are supposed to be made of smaller forts and earthworks.
