What is the downside of creating naval warship stacks and sending them to the shipping/blockade boxes or across the oceans for months on end? Do they lose cohesion or health after a while and have to be returned to port or can they stay at sea for years ? I've not got into the naval side much yet.

If you have naval box handling set to normal, they will lose cohesion and supplies. Supplies will drop very quickly and consequently so will cohesion, but put a few transports with them and your patrol stacks can stay out for a good number of months.
