I'm not 100% on this, but I think if Fort Sumter doesn't fall to the CSA, he won't trigger.

Well, Sumter has fallen to the Confederates and he still hasn't appeared as of early August 1861. What gives?

Conditions
MinDate = 1861/04/12
MaxDate = 1861/06/30
Probability = 50

Meaning, that every turn between the specified dates the odds are 50%

So I'm assuming that if he hasn't appeared by MaxDate = 1861/06/30 he isn't appearing at all? If so, why?

Because the event is written like that.
When the engine reads the Conditions section of the event and game date is outside min/maxdate, it will not execute it.

So, no, it will not appear anymore, unless you alter file USA Events.sct or write an own event.

I've only had it happen once in maybe 7 or 8 turns playing as the Union. It doesn't seem to be a regular occurrence.
