That's weird, the region is strictly identical to other Prussian regions in terms of National status.

If you check the tooltip, it says that it might happen if the region is cut from the capital or if you control it but did not get it with a peace treaty. Does it apply here?

No, I am playing as Prussia, Hamburg is controled by Prussia right from the beginning - but just checking it: you are right.

Hamburg has got no direct connection to the main Prussian territory. It can be reached by a short passage through Holstein or through Hannover, the necessary passage rights are given. Therefore I didn't notice Hamburg was isolated.

Good! That's done this way, because any region in national territory can propose the full national stockpile of merchandises to buyer, and we really don't want that allowed for distant regions, cut from the capital.
