done it a few times. There are two solutions and you need to pick your route: a) embrace it, reduce culture, pick up the -5 tokens, get the revolt, revert to stable/tier II and rebuild. meant to add, don't forget that the ratio of military buildings has an effect too. So if you are below 1, it'll reduce the average quality, get above 4 and you get another improvement in the chance to get better leaders

It does - but its random. The % is in the game files (I don't have them on this laptop so can't double check). You'll still get complete dross even with a couple of these buildings, but you also get more so statistically better chance of at least some decent commanders.

you can find them in the buildings database. They are roughly in clusters in Greece/Asia Minor, Egypt, Babylonia and then some scattered about NW Europe. For Rome, I think they tend you to a Greece-first strategy, at the moment there is rarely much of a reason to go after Carthage. Ok weakening them...

Pocus answered this point in an another thread. Basically its a good idea that demands programming time to implement.

You can make some rough and ready guesses - since a heavy/medium infantry unit is around 5,000 men (a legion), call a cavalry or light infantry unit 2,500 ... good enough.
