Guess you are right. I got to 1632 and only saw money from selected big cities or whole regions (Bohemia, Bavaria). However, the game has no message about taxes/money from controlling the entire Palatinate. I control it all but only get money from Frankfurt.

Yes, Bremen can become active, though I do not think any message notifies you about it.
